New patients do not require a referral.  Prospective patients should contact the office directly at 515-270-2242 to schedule an appointment. At the time of scheduling, we will go through a brief phone assessment to evaluate your needs and find an available appointment with the provider of your choice.

A credit or debit card is required for all NEW appointments. This is to guarantee the appointment time, and the card would only be charged in the event of a no-show or last-minute cancellation/reschedule outside of a 24-hour notice.
